Agri-Fab Mail-In Rebate Instructions:

  1. Purchase the Agri-Fab SmartSWEEP™ (Model 45-0456), SmartLINK™ Master Platform (Model 45-0453) or SmartCART™ (Model 45-0464) between April 1, 2009, and June 30, 2009.
  2. Send:
  1. The original mail-in rebate form filled out completely and clearly including name, address (no P.O. Boxes), city, state and ZIP code.
  2. The original dated cash register receipt with the store name and the Agri-Fab SmartSWEEP, SmartLINK Master Platform or SmartCART product circled.
    Fill in the UPC number(s) on the printed rebate form. UPC numbers can be found on the outside of each Agri-Fab product.
  3. Mail to: Agri-Fab Rebate, P.O. Box 3682, Dept. 131118, Medina, OH 44258-3682.
  4. Receive by mail a $20 rebate check.

Please allow 6–8 weeks for shipment of rebate check. Offer valid on purchases made by 6/30/09. Claims must be postmarked by 7/31/09 and received by 8/15/09.
