To receive your Agri-Fab® rebate check by mail, follow the steps below:
1. Make any single store or online Agri-Fab purchase totaling $200, or more, excluding tax, between March 17, 2008 and June 1, 2008.
2. Send this original completed mail-in rebate form and the original, dated invoice or cash register sales receipt, which includes the store name,
if applicable, and the purchase price of $200.00 or more of Agri-Fab products circled, to the address below.
3. Mail rebate submission to: Agri-Fab Rebate, P.O. Box 3693, Medina, OH 44258-3693
4. Submissions must be postmarked by 6/30/08 and received by 7/18/08.
Please allow 6-8 weeks for shipment of rebate check.
